I'm trying to figure how this would have occurred ..... was the concrete still in the forms?

If this happened shortly after the concrete was poured and during the drying stage then the extra water that somehow washed out that section has compromised the strength of concrete.

Too much water in the mix will dilute the cement and greatly reduce the strength of the concrete mix in that area.

Also a 3 foot wide and 18 inch tall hole is huge and will require some sort of re-enforcement.

This sounds like a structural engineer is needed to evaluate the situation and recommend a proper repair.
