Where are you located? An area that recently experienced a hard frost(frost heave). What soil type do you have (expansive soil)?

Otherwise suspect rain run-off path from the stoop or overhang (errosion) and/or expansive soil which expands and contracts and inconsistant soil moisture. You said nothing about how this added concrete (footings or just a slab, base, depth, etc.) if done correctly, suppose could be aggrivated due to animals digging/burrowing.
Another thought - underground leak of sewer or water line (main or irrigation system) causing a sink hole.
Hard to tell what your describing (and not describing), a picture would be helpful too.
