It sounds like you have the white wire to the fan connected with the black wire to the lamp and the wire from the lamp switch.
You should connect all three white wires together, and only the wire from the fan switch to the black wire to the fan, and only the wire from the lamp switch to the black wire to the lamp.
In some units, especially older ones, all the wires may be black. In that case, one wire to the fan and one to the lamp should connect to the white wire coming from the switch. Then the wire from the lamp switch connects to the remaining wire to the lamp, and the wire from the fan switch connects to the remaining wire to the fan.
